The bizarre moggy, bearing the double-barrelled moniker Frankenlouie to account for both cats Frank and Louie, died at the age of 15.
The cat's owner, Marty Stevens, adopted the strange creature after he was brought into a veterinary school where she worked.
At a day old, he was about to be put down but Ms Stevens took pity on him and brought him home.
In the video made before his death, Ms Stevens said: "Frank does the eating, Louie just hangs out."
He was the longest surviving member of a group known as Janus cats which were named after a Roman god with two faces.
Janus cats are unlikely to survive as most have congenital defects, but Frankenlouie made it into the Guinness World Records for being the oldest on record.
The Telegram of Worcester reported that the pet died of cancer on Thursday at the Cummings School of Veterinary Medicine at Tufts University in Massachusetts.
